Community Health in Resource-Limited Settings

Community health education in underserved communities in Costa Rica and Ecuador, in Spanish. February and July every year.
Training in a well-resourced system teaches you to reach for the test. Spending time where the test is not available teaches you something else, and most clinicians who have done both say the second changed them more.
This is not a clinical program. You are not placed in a hospital or clinic, and you do not observe or assist in patient care. The patient-facing work here is community health education. You work directly with community members, in Spanish, on health topics those communities identified.

Who This Is For

Medical, nursing, PA, NP, pharmacy and public health students, and practicing clinicians. Students planning careers in underserved medicine, rural practice, community health centers, or global health.
There is no Spanish minimum. Instruction is set to your level.

Timing and Length

When

Twice a year, February and July.

Length

Three and four week programs within a four-week block.

What the Program Includes

Documentation

Documentation of hours and activities.

Community health education

Direct work in communities on the conditions they live with: elevated blood sugar, elevated blood pressure, malnutrition. These topics are chosen by the communities and the partner organizations, not by us and not by you.

Medical Spanish instruction

Roughly 20 hours a week, oriented toward patient communication and health education.

Host family lodging

One student per household. You live in the community, not adjacent to it.

What Resource-Limited Actually Means Here

It does not mean chaos, and it does not mean a population waiting for outside help. Costa Rica has one of the strongest public health systems in Latin America. Ecuador has functioning clinics staffed by capable clinicians. What is limited is equipment, imaging, medication availability, and specialist access.
So the learning is specific, and it comes from the community rather than from a ward. You see what people manage at home because the alternative is unavailable. You see which health problems go unaddressed until they are serious, and why. You see what a family does when the nearest specialist is four hours away.
Students headed into underserved practice in the United States tend to say this is the part that transferred.
On the framing. We are careful with the phrase resource-limited because it can slide into a narrative where visitors arrive as the resource. They are not. The clinicians and community health workers you learn from manage these constraints every day and will continue to after you leave.
