Fourth-Year Medical School Electives: How to Choose
Fourth year is the first time your schedule is largely yours. This guide covers what the elective categories are, how to think about choosing, and where an international elective fits.
The Main Categories
Audition or away rotations
Rotations at institutions where you hope to match. These function as an extended interview and are the highest-stakes elective you will do. Scheduled through VSLO in most cases, with deadlines that arrive earlier than students expect.
Sub-internships
Acting-internship rotations where you carry responsibility closer to that of a first-year resident. Most schools require at least one. Many students do a second in their intended specialty.
Specialty exploration
Rotations in fields you are considering but have not committed to, or fields you will not train in but will refer to constantly.
Skill-building electives
Radiology, critical care, ultrasound, procedures. Chosen for what you will need on day one of residency rather than for what you will specialize in.
Research or scholarly electives
Protected time for a project, useful for competitive specialties and for anyone considering an academic career.
International and global health electives
Clinical or public health work abroad, usually with a language or health-systems component.
Board study time
Not an elective, but it has to fit in the calendar, and students who plan for it late regret it.
How to Actually Choose
Work backward from match day
Audition rotations belong early, typically July through September, so letters arrive in time. Everything else fits around that.
Be honest about which rotations are for the match and which are for you
Both are legitimate. A student who fills the entire year with strategic rotations and none they wanted usually says so afterward.
Protect interview season
October through January is heavily disrupted. Put demanding rotations outside it and lighter ones inside it.
Take the thing you will never have time for again
Fourth year is the last stretch of protected time before residency. Whatever it is you have been meaning to learn, this is when.
Where an International Program Fits
International programs usually make sense in the second half of the year, after audition rotations and outside interview season, though February and July programs can both work depending on your schedule.
Two things worth knowing before you commit.
Approval is not automatic
Your school decides whether an international program counts and what it counts as, and requirements differ. Programs built around language instruction and community health are often credited differently than clinical rotations, or not at all. Check with your dean’s office or elective coordinator before you enroll anywhere.
The quality range is wide, and the clinical claims are where to look.
A number of international programs sell placement in hospitals and clinics abroad, with observation or hands-on experience beyond a visiting student’s scope. That is an ethical problem, and it is also something that will not survive scrutiny in a residency interview. Ask any provider directly whether students are placed in care settings, and be careful with the ones who say yes.
What We Offer
We run Spanish immersion programs in Costa Rica and Ecuador in February and July, built around roughly 20 hours a week of medical Spanish instruction and community health education work with local partner organizations. Programs run three to four weeks and we provide documentation of hours and activities.
These are language and community health programs. We do not place students in hospitals or clinics, and there is no clinical observation component. What you take home is Spanish you can use with patients and a working understanding of how another country organizes health for its population.
